(Your Name) is a 39 year old male with a spinal injury at the T4-5-6 level on 10/93. He is status post-spinal surgery. He presents with a spastic paraplegia, neurogenic bowel and bladder. Due to his diagnosis, he is at high-risk for osteoporosis, decubitus ulcers, constipation and urinary tract infections, soft tissue and bony contractures as well as circulatory problems amongst other problems. Due to his wheelchair bound status, he is also limited as far as job opportunities, recreational and social activities.
(Your Name) is independent in locomotion with a manual wheelchair. He is also independent for all ADL, transfers and driving an adapted van with hand controls. His upper extremity strength is within normal limits. He is unable to stand at this time. He holds a job as a disability counselor which requires both in-office and out of office work. His in-office work is limited at this time by his inability to stand in order to access standard equipment and furniture such as above wheelchair level shelves and cabinets. At this time I am recommending the LifeStand standing aid device/wheelchair reference #00000000, 18 inch 700 Newton gas cylinders with no flat inserts for the following medical and vocational reasons:
To prevent osteoporosis by allowing weight bearing on the lower extremity bones.
To prevent spontaneous fractures or fractures caused by minor trauma due to osteoporosis.
To prevent decubitus ulcers from excessive pressure from prolonged sitting.
To decrease spasticity and thus prevent soft tissue contractures.
To improve physical balance and psychological well being.
To assist with digestion, intestinal transit and therefore bowel program.
To improve blood circulation.
To improve renal and bladder function.
To decrease the number of daily transfers.
To increase vocational skills and thus work opportunities.
To improve socialization.
To increase recreational opportunities.
