To Whom It May Concern:

(Your Name) is a 37 year old male with a diagnosis of T-10 paraplegia secondary to a spinal cord injury, which occurred in 1974. At that time, he also suffered several lower extremely fractures. Subsequent complications from this injury have included a sacral decubitus for which he was bedridden for several months in 1990 and recently diagnosed osteoporosis affecting his spine and lower extremities.

(Your name) is independent at wheelchair level using a manual wheelchair. He is unable to do any ambulation at this time. He works full-time as a drafter which he is able to perform at wheelchair level. His strength and range of motion in both upper extremities is within normal limits. In his lower extremities, he has no active movement. Dorsiflexion in both ankles is limited to zero degrees, but his range of motion is otherwise within normal limits in the lower extremities. Both lower extremities are essentially flaccid, however, he does have occasional spasticity in his quadriceps and plantar flexors.

(Your Name) was recently seen in physical therapy to evaluate his ability to utilize both a tilt table and a standing frame. He tolerated being upright with either of these pieces of equipment for 20-30 minutes and was able to transfer on and off of both pieces independently. He did require assistance with the hydraulic lift of the standing frame, but it is felt this was due to the location of the controls rather than to any limitations in his abilities.

It is recommended that (Your Name) use a portable standing device for the following reasons:

1. It will enable him to prevent future skin breakdown by providing a means of pressure relief.2. Being in a standing position will help him better be able to control his spasticity and prevent further contracture at his ankles.3. Being in a standing position will enable him to weight bear through his lower extremities and spine, thus helping to prevent further bone density loss.4. A portable unit would enable him to use the device while he is at work, thus allowing him to perform functional activities while he is in a standing position.
