To Whom It May Concern:

(Your Name), is a gentleman with a primary diagnoses of paraplegia who has been under my care for treatment of a recurrent right trochanteric decubitus ulcer. (Your Name) has recently had surgery for closure of this decubitus. (Your Name) is wheelchair dependent, secondary to his paraplegia.

I have recommended to (Your Name) that he purchase a LifeStand Wheelchair. This wheelchair would allow him to place himself in a standing position. This is necessary in order to prevent pressure that occurs in a seated position. This will help with (Your Name) to prevent any future pressure sores. This wheelchair is also convertible to a standard seated wheelchair. This wheelchair is medically necessary both for (Your Name)ís mobility and also for prevention of any future pressure sores.
