To Whom It May Concern:
This letter is in regard to my former patient (Your name) is a 45 year old female who was referred to me with a diagnosis of Multiple Sclerosis (MS). She has required a wheelchair for mobility and maximum assistance of family members for bed mobility, and wheelchair to bed transfers. Her muscle tone was significantly decreased in her lower extremities and her right lower extremity strength was generally +1/5, left lower extremities 0/5. She had functional strength in her upper extremities, normal sensation throughout all 4 extremities and fair sitting balance at the time of discharge.
I am writing this letter to recommend the LifeStand Wheelchair for (your name). I believe it would benefit her in a number of ways:
1. Regarding physical functional it would help to improve her muscle tone and balance and facilitate remaining strength in her trunk and lower extremities. It may also make it easier for her to perform functional activities which she had not been able to do thus far, such as dishwashing, meal preparation, reaching higher areas in the house which would benefit her upper extremity strength/function. It will make it easier for family members, particularly, elderly care givers, to assist her in transfer activities.
2. Regarding (your name) overall health, it would help to prevent pressure areas in the buttocks and thighs. Reduce her significant low back pain, improve digestion, bowel function, circulation, and reduce the risk of osteoporosis, all possible scenarios with prolonged wheelchair use, thus reducing possible future medical expenses.
3. It would also provide an improved sense of well being which is crucial for many wheelchair users in the ability to stand and met people eye to eye.
(Your name) is very motivated to remain as functional as possible. I believe she could increase her function with the LifeStand Wheelchair.
Sincerely,
Your Physical Therapist
